Treatment Comparison

Emface vs. Botox: Which Facial Treatment Is Right for You?

They are constantly compared, but they do opposite things to your facial muscles. Understanding that one difference makes the choice obvious — and explains why so many patients end up using both.

Emface treats the forehead and cheeks hands-free, with no needles — one of the clearest practical differences from Botox.

The short version

  • Botox relaxes a specific muscle so it stops creasing the skin above it. Best for expression lines.
  • Emface strengthens the muscles that lift your face, while radiofrequency firms the skin. Best for sagging and lost contour.
  • Botox is repeated every three to four months. Emface is a short series, with results that typically last up to a year.
  • They are not rivals. Treat structure with Emface, then refine expression lines with Botox if any remain.

If you have researched facial rejuvenation in the last year, you have almost certainly seen Emface and Botox mentioned in the same sentence — usually as if you have to pick one. That framing is misleading, because the two treatments are not different routes to the same destination. They act on your facial muscles in opposite directions.

Botox works by reducing muscle activity. Emface works by increasing it. Once that lands, choosing between them stops being a matter of which is trendier and becomes a straightforward clinical question: is the thing that bothers you caused by a muscle doing too much, or by your face losing the support that holds it up?

Below we walk through what each treatment actually does, where each one genuinely outperforms the other, and how we decide between them at Aesthetic Wellness at University Dental Group here in Orlando.

What Botox does — and what it does not

Botox is a purified botulinum toxin injected in very small, targeted doses. It temporarily blocks the nerve signal that tells a specific muscle to contract. When the muscle stops pulling, the skin folded on top of it relaxes, and the line it was creating softens.

That mechanism makes Botox excellent at a narrow set of problems — and largely irrelevant to others. It is a precision tool for dynamic wrinkles: the lines your face creates when it moves.

  • Frown lines between the brows, from repeated squinting or concentrating
  • Horizontal forehead lines, from raising the brows
  • Crow's feet at the outer corners of the eyes
  • A "lip flip" or gummy smile, where a small dose changes how the upper lip sits

What Botox does not do is add support. It cannot lift a cheek that has descended, sharpen a jawline that has softened, or rebuild collagen in thinning skin. In fact, relaxing a muscle does nothing for laxity — and if the underlying issue is loss of structure, Botox alone tends to produce a face that is smooth but still looks tired.

This is the most common disappointment we hear about in consultations: a patient who has had Botox for years, likes the effect on their forehead, and cannot understand why their face still reads as heavy or drawn. Nothing went wrong. The treatment was simply aimed at the wrong layer.

What Emface does differently

Emface by BTL is a non-invasive, FDA-cleared treatment that delivers two synchronized energies at once, hands-free, with no needles:

  1. Synchronized radiofrequency gently warms the deeper layers of the skin, stimulating fresh collagen and elastin. This is the firming, smoothing half of the treatment.
  2. HIFES™ muscle stimulation (High-Intensity Facial Electrical Stimulation) triggers thousands of supramaximal contractions in the muscles that elevate your face. This is the lifting half.

The second mechanism is the one with no equivalent anywhere else. Your face has muscles that work as elevators — they hold your brows, cheeks and midface up against gravity. Like any muscle, they lose tone over time, and nothing in a standard aesthetic menu trains them. Emface does, in roughly twenty minutes per session, while you lie still.

Botox asks a muscle to stop working. Emface asks a muscle to get stronger. When a patient's real problem is descent rather than creasing, that distinction decides the outcome.

Because it treats skin and muscle together, Emface addresses the two things that actually drive the aged look: lost structure and declining skin quality. Results appear gradually rather than overnight, which is exactly why they tend to read as "you look well" rather than "you had something done."

Which one fits your goal?

Rather than comparing treatments, describe your concern in your own words. The right answer usually follows immediately.

"The lines between my brows make me look angry."

That is a dynamic wrinkle from an overactive muscle. Botox is the more direct tool, and it will work faster than anything else on this page.

"My face looks like it is sliding downward. My cheeks are flatter than they were."

That is descent and loss of support, not creasing. Emface is the appropriate treatment, because it rebuilds the structure underneath rather than smoothing the surface.

"My jawline has gone soft and I have a bit of a double chin."

Emface, often with Exion added to tighten skin along the jaw and under the chin. Botox does very little here, because the issue is laxity rather than muscle overactivity.

"I am in my thirties and want to stay ahead of it."

Emface as preventative care makes good sense — maintaining muscle tone and collagen is considerably easier than restoring them later. Many of our younger patients start here and add nothing else for years.

"I already get Botox and I like it, but something is still missing."

This is the single most common reason patients book an Emface consultation, and the answer is usually not to stop the Botox. It is to add the layer the Botox was never going to reach.

Can you combine Emface and Botox?

Yes — and we offer both in-house, which makes the sequencing straightforward.

The approach we generally recommend is structure first, refinement second. Complete your Emface series so the lift and firming have taken hold, then reassess. Some expression lines soften on their own as skin quality improves. Whatever remains can be treated with a small, precisely placed dose of Botox — often less than the patient was using before.

Doing it in that order matters. If you start with a full round of Botox and then begin Emface, you are relaxing muscles in some areas while training them in others, which makes it much harder to judge what each treatment contributed. It also risks paying for units you did not need.

One practical caution: because timing and placement interact, both treatments should be planned by the same provider. Booking Emface at one practice and Botox at another is how patients end up with results that fight each other.

What a consultation in Orlando looks like

Our facial aesthetics practice sits inside a dental office, and that is a genuine clinical advantage rather than a quirk of scheduling. We already assess your jaw muscles, your bite and your tension patterns as a matter of routine. When a patient arrives with a heavy jawline, tension headaches or asymmetry, we are reading the whole system, not just the skin.

A consultation takes about half an hour. We look at your face at rest and in motion, ask what specifically bothers you, and tell you honestly which treatment addresses it — including when the answer is "neither of these" or "less than you were expecting to need." You will leave with a written plan and exact pricing, with no obligation to book treatment that day.

If you are weighing Emface against Botox and cannot tell which describes your situation, that ambiguity is itself a good reason to come in. It takes a few minutes in person to resolve something that is genuinely difficult to settle from photographs online.

Frequently asked questions

Is Emface better than Botox?
Neither is universally better — they solve different problems. Botox relaxes a specific overactive muscle to soften the wrinkle it creates. Emface strengthens the muscles that lift the face and stimulates collagen in the skin above them. If your main concern is a deep expression line, Botox is the more direct tool. If it is overall sagging, loss of contour or a tired-looking face, Emface addresses the underlying support.
Can you have Emface and Botox at the same time?
Yes, many patients use both, and we offer both in-house. The usual approach is to treat structure first with a series of Emface sessions, then use Botox selectively for any expression lines that remain. Timing and placement matter, so both should be planned by the same provider rather than booked independently at different practices.
Does Emface hurt compared with Botox injections?
Emface involves no needles at all. Applicators rest on the forehead and cheeks, and most patients describe warmth plus a tapping or twitching sensation as the muscles contract. Botox involves several quick injections with a very fine needle, which most people tolerate easily but do feel. Neither requires anesthesia.
How long do Emface results last versus Botox?
Botox results typically last three to four months before the muscle regains activity and treatment is repeated. Emface results build over the weeks following your final session and typically last up to a year, with many patients maintaining them through periodic touch-up sessions.
Is Emface FDA-cleared?
Yes. Emface by BTL Industries is FDA-cleared and combines synchronized radiofrequency with HIFES™ muscle stimulation. It is non-invasive, uses no needles or incisions and requires no recovery period, so patients return to normal activities immediately.
Which treatment is better for a double chin or jawline definition?
Botox does little for a soft jawline because the issue is skin laxity and lost support rather than muscle overactivity. Emface addresses the submentum and jawline by firming skin and re-toning the muscles that support it, and Exion can be added to tighten skin along the jaw and under the chin. A consultation determines which combination fits your anatomy.

Emface, Exion and injectable treatments are not appropriate for everyone, and individual results vary. Suitability, session counts and expected outcomes can only be determined during an in-person evaluation of your health history and facial anatomy.
